If your doctor, dentist, or other provider doesn’t offer direct billing, you’ll need to:

Here’s how to submit your claim:

π Choose the correct claim form (Health, Dental, Vision, etc.)

π Fill out the form and attach your receipt

πΈ If you want faster payment, complete the Direct Deposit Form

π§ Email your completed forms to acadia@medavie.bluecross.ca or follow the mailing instructions on the form
Tip: For dental claims, you can ask your dentist to submit the form electronically using your Student VIP benefits card.
