📂 Choose the correct claim form (Health, Dental, Vision, etc.)

💸 If you want faster payment, complete the Direct Deposit Form

📧 Email your completed forms to acadia@medavie.bluecross.ca or follow the mailing instructions on the form
Tip: For dental claims, you can ask your dentist to submit the form electronically using your Student VIP benefits card.
