Please complete this form if you wish to withdraw from an Open Acadia online course. If you are withdrawing from a Fall or Winter remote delivery course, please withdraw through Acadia Central.

Students withdrawing from Acadia must do so by completing this Notice of Withdrawal. Students ceasing studies without notification are ineligible for fee adjustments.

An extension for online course may be requested one time. Approval is at the discretion of the course instructor. You may request extensions for up to four courses using this form.
