Please complete this form if you wish to withdraw from an Open Acadia Continuous Intake course. If you are withdrawing from a Fall or Winter remote delivery course, please withdraw through Acadia Central.
An extension for Continuous Intake courses may be requested one time. Approval is at the discretion of the course instructor. You may request extensions for up to four courses using this form.
